The Jefferson Center for Connected Care is a University-based research center focus on developing and testing innovative solutions toward a more patient-centered care delivery system. While portfolios within the Center are diverse, they align at the high level through their focus on addressing issues of health disparities and gaps within the health and social systems that prevent people from achieving desired outcomes. Examples portfolios include food is medicine, digital health equity, harm reduction for people who use drugs, patient uncertainty, and building models for decentralized care.
