Abstract
Social determinants of health (SDOH) have a greater impact on health outcomes than clinical care. Gastrointestinal (GI) diseases are a major source of health care utilization and costs in the U.S., totaling $111.8 billion in 2021. SDOH, grouped under Healthy People 2030 SDOH categories, were considered for their possible impact on six common GI diseases. Subsequently, a 3-question survey was sent to 41 gastroenterologists, who were asked to complete the survey and also forward to a partner. There was a 60% (49/82) survey completion rate. Of respondents, 76% were “Very aware” or “Aware” of Healthy People SDOH; 84% noted that addressing SDOH in GI care was “Very important” or “Important;” 59% noted that addressing SDOH regarding GI business aspects was “Very important” or “Important.” While community gastroenterologists understand the importance of SDOH and clinical care, the potential impact of SDOH in GI business aspects is less appreciated. Utilizing a validated SDOH screening assessment, Health Leads Social Needs Screening Toolkit, one may implement an SDOH screening and assessment program within a GI practice. It is important to code the ICD-10 Z codes for SDOH. The business case for screening and addressing SDOH includes improved health outcomes, reduction of healthcare utilization and costs, and improved patient experience. Specifically, in a GI practice, one may decrease no-show rates in the office and endoscopy suite; increase screening rates for potentially preventable cancers; increase access to medications; increase diagnostic and treatment adherence; maximize fixed costs and improve variable costs by increasing fill rates; improve GI health outcomes; and promote value-based care. Using Kotter’s 8-stage process of creating change for SDOH program implementation, a 6-step proposed pathway for assessing and addressing SDOH in GI care is presented. Raising awareness of the role of SDOH in GI care at the American College of Gastroenterology is also discussed.
