Abstract
Background: Home-Based Palliative Care Programs offer a value-based care opportunity to improve quality of life and decrease resource utilization while promoting care that aligns with a patient’s values and preferences.
Purpose: Understand core elements of value-based home-based palliative care programs that can be reproducible to organizations.
Methods: Literature review conducted of three home based-palliative care programs: Agilon Health, ProHEALTH, and Sharp Healthcare.
Results: Each program demonstrated that value based palliative care interventions led to reductions in utilization and cost through less ED and hospital admissions. Components of successful programs include multidisciplinary teams, 24 hour/7 day a week access to care, primary care provider involvement, primary care clinician education in essential elements of palliative care.
Conclusion: Integrating palliative care and primary care can provide goal alignment in a full risk model to improve patient outcomes and cost reduction.
