Abstract

Pharmacists are considered among the most trusted health professionals, yet their role in population health must be clearly defined and established. Population health teams aim to improve the health outcomes of a group of individuals. Pharmacists play a role in developing evidence-based disease management programs and medication management and assisting with closing medication-related CMS quality Stars measures. Success in adding a pharmacist to the team can be demonstrated by return on investment and will ensure senior leadership buy-in. Closing the Stars measures can have a financial impact as the higher the overall star rating, the higher the reimbursement for the health plan, and better outcomes will be achieved. Population health pharmacists have a role beyond the care team and can grow as part of the leadership team. Achieving the goal of adding pharmacists requires looking at department structure, leadership structure, and the particular work they complete to show value. A review of the state pharmacy laws must be completed, as many states allow pharmacist-collaborative agreements. This can help lead to better results as pharmacists will be able to change medications as needed. Overall, depending on the state laws and population health department structure, they can perform many activities to improve a population's health outcomes.
