Abstract

Addressing social determinants of health is a primary approach to achieving health equity. Join us as we explore the work of the Philadelphia Collaborative for Health Equity, The Frazier FamilyCoalition for Stroke Education and Prevention and our partnership with the Temple Center Population Health. In this session we will share program strategies to improve population health in North Philadelphia; discuss health system approaches to advance health equity; and explore the role of Community Health Workers in the effort to reduce health inequalities.
