Abstract
Despite decades of effort in the field of patient safety, there are as many as 400,000 patient deaths per year from preventable harm. The concept of high reliability (HRO) has been shown to prevent accidents even in areas of operational complexity. This talk is about Sentara Healthcare’s journey towards improved safety through high reliability principles and how the system overcame the lack of sustainability through inculcating principles and tools into standard work.
