Utilizing HEOR Techniques and Competencies in Evaluating U.S. Healthcare Policies and Programs: A Case Study on the U.S. Food and Drug Association Accelerated Approval Program in Oncology

Abstract
Health Economic and Outcomes Research (HEOR) utilizes techniques and competencies to help decision makers evaluate the effectiveness of treatments in specific circumstances and determine access. While these techniques and competencies are used in some policy areas—cost effectiveness and cost-utility analyses, and patient-reported outcomes, for example—they are usually used in a specific drug/health technology situation or in evaluating a specific policy intervention (e.g., Covid vaccine mandates). The HEOR approach can and should be utilized as an evidentiary basis for decision makers to adopt new or change existing healthcare policies and programs that also have significant impact on patient access. The objective of this capstone was to demonstrate how HEOR techniques can evaluate U.S. healthcare policies. The methodology used was to critically evaluate recently published novel research on the FDA’s Accelerated Approval Program (AAP) in terms of using HEOR competencies to evaluate a healthcare policy. The evaluation found that HEOR competencies (counterfactual model, statistical practices, data extraction and assumptions) effectively estimated that the AAP did add life years to oncology patient lives (263,000 life years across 69 indications for approximately 911,000 patients from 2006 to 2022), and therefore helped ground discussion around the debate around the program in evidence at a population health level. In conclusion, policy decision makers must prioritize valid methodology and evidence in evaluating policies, and HEOR capabilities can assess the utility and effectiveness of healthcare policies. Using a critical evaluation of a study on the FDA AAP, this capstone demonstrates that HEOR methods can help evaluate U.S. healthcare programs and ground policy discussions in evidence.
