Incorporating the Updated Hyperbilirubinemia Guidelines into a Busy Inpatient Pediatric Unit

Abstract
In August 2022 the American Academy of Pediatrics updated the clinical care guidelines for the care of hyperbilirubinemia in infants born at 35 weeks gestation or greater. The AAP then announced that there would be a national collaborative project aimed at incorporating these updated guidelines into inpatient units (newborn nurseries, neonatal intensive care units, and inpatient pediatric units) across the country. This project was aimed at incorporating these updated guidelines into an inpatient pediatric unit while also participating in this national collaborative. The team used quality improvement methodology to perform PDSA cycles throughout the process. The data was tracked both internally and was entered into the national database as part of the AAP collaborative project. Outcomes of the project included a reduction in the percentage of patients readmitted for subthreshold phototherapy and a reduction in the overall number of admission for hyperbilirubinemia. To ensure no harm occurred as a result of the project, the number of patients meeting escalation of care was tracked with no measurable increase detected over the duration of the project.
