Abstract
In 2002 Sentara Healthcare began a journey towards improved patient safety through implementation of high reliability principles. Serious safety events initially decreased. Over time there was a lack of sustainment evidenced by increases in preventable harm events and it was imperative to refresh the model. The focus of the project was around again instilling high reliability principles across the organization. Four interweaving initiatives were explored as pathways towards providing safer care: Revised safety toolkit, standardized safety orientation, incorporation of specific high reliability organizing tactics into standard work, and clinical variation reduction. These changes have led to improved clinical outcomes.
