On October 21, 2009, The Jefferson School of Population Health hosted a 1-day policy forum, Diabetes Management: Building a Coordinated Care Model". The Forum brought together over 100 leaders in health care, including providers, educators, legislators, business leaders, media and others to discuss coordinated care models for patients with diabetes.
The program discussed health consequences and socio-economic impact of diabetes in the United States and current and future trends and standards of care in the management of diabetes.
