Abstract
Objectives
- Demonstrate ways to engage patients who are experiencing homelessness and have mental health diagnoses and/or substance use history who encounter inpatient hospital settings
- Discuss different barriers that these patients may face when encountering more formalized hospital or hospital-like settings
- Understand how diagnostic overshadowing and allostatic load affects healthcare delivery to patients experiencing homelessness
- Recognize how trauma-informed practices can increase patient engagement and reduce self-directed discharges
- Expand knowledge on importance of care coordination in in-patient hospital settings
- Demonstrate the importance of nurse-led continuity of care via warm hand-offs
- Learn now you can take the information from our program to advocate for Street Med patients in your current or future hospital systems
