In support of the Know Diabetes by Heart™ (KDBH) initiative, the American Diabetes Association has assembled a collaboration of healthcare systems and stakeholders in Philadelphia and the mid-Atlantic region to improve population health outcomes for people with diabetes with or at risk of cardiovascular disease via its Diabetes INSIDE® quality improvement initiative. The QI Initiative is a 3-year effort that engages healthcare systems with QI training, coaching and facilitation, data analysis, data-driven professional education, shared learning, and guideline support.
