The Jefferson School of Population Health held a one-day interactive educational program focused on improving outcomes for patients with diabetes
The program was held on Wednesday, September 29, 2010 at The Chemical Heritage Foundation Conference Center at 315 Chestnut Street in Philadelphia.
This event was a follow-up to last fall’s program, which explored innovative care models under consideration for people with diabetes. For this program, we brought together key stakeholders (patients, providers, employers and payers) to engage in a practical, interactive dialogue on how to organize, implement and finance a coordinated care model that will maximize improvement in patient outcomes.
