• Quality evaluations (surgical and otherwise) show gaps and unwarranted variability.
• So, QI is needed. Yet it remains difficult to do well, and studies show it is often poorly performed.
• Frontline QI by clinical surgeons has opportunities. I propose a way to "improve" improvement (EPoSSI).
