Abstract

Background: Ankle sprains are among the most common musculoskeletal injuries across all patient groups. Structured rehabilitation with resistance bands decreases re-injury by improving balance, range of motion, and strength, but protocols vary widely. Furthermore, patients often struggle to perform resistance band exercises correctly, limiting their effectiveness.

Aim: To design and develop a wearable rehabilitation device that standardizes resistance band therapy for ankle sprains by controlling resistance vectors and progressive overload.

Methods/Design: We conducted a needs assessment of current rehabilitation practices and identified limitations in resistance band use, including lack of standardization, variability in patient adherence, and difficulty replicating exercises at home. Guided by these findings, we designed a wearable ankle rehabilitation tool that fixes resistance vectors and quantifiably measures progression of strength. In addition to standardizing therapy, the device simplifies patient use—rehabilitation is hands- free and requires less provider supervision.

Results: We designed a machine-washable sleeve that wraps around the ankle. Resistance bands clip into anchor points across the sleeve and tighten to modulate resistance. After sufficient strengthening, the user can swap in a thicker band. Fabric loops guide the bands along predefined tracks, stabilizing the vector of resistance during movement.

Anticipated Impact/Conclusion: This innovation addresses the unstandardized exercise mechanics and poor patient adherence of ankle sprain rehabilitation. By addressing critical gaps in ankle sprain rehabilitation, our device has the potential to reduce re-injury rates and long-term instability. Future studies will evaluate optimal strengthening protocols, patient adherence, and whether the device decreases the need for in-person therapy.
