Abstract

Introduction/Purpose: Somatic genomic testing in metastatic prostate cancer (mPC) identifies genomic variants which provide clinical insight into the disease biology. Somatic testing results determine eligibility for standard of care targeted therapies and clinical trial eligibility. While national guidelines recommend testing for all mPC patients, patients with adverse social determinants of health (SDOH), racial minority groups and patients with low socioeconomic status (SES) have historically undergone lower rates of testing compared to national standards. This study aims to identify barriers to testing in mPC patients in the Jefferson system and ultimately understand the factors that influence testing rates.

Methods: Epic’s clinical database was queried using diagnosis, procedure, lab, and encounter codes to identify 128 patients with mPC within the Jefferson system along with somatic genomic testing status. Importantly the query was limited to patients with an encounter in 2024. Iterative cohort generation, chart review validation, and refinement was required to ensure patients met the inclusion criteria. Additional data elements including patient demographics, SDOH variables, and other clinical data were added to the cohort. We plan to analyze these data to determine if there are any associations between mPC patients’ testing status and demographic or SDOH variables.

Results/Conclusions: The results of the data analysis are pending and will be presented in future work. We anticipate the results will show that patients from ethnic or racial minority groups or who have adverse SDOH are less likely to receive somatic genomic testing in accordance with guideline recommendations
