Abstract
Introduction/Purpose: Chronic pain is common in OI patients, but standardized pain assessment and treatment is lacking. Pamidronate, a bisphosphonate traditionally administered to OI patients to increase bone mineral density, could play a role in pain management. This study will describe baseline pain before receiving pamidronate to contribute to the characterization of pain in OI and justify the need for evaluating the impact of pamidronate on pain over time.
Methods: 54 patients with mild to severe OI were selected from the Nemours Skeletal Dysplasia Program (mean age 10.5±6.6 years [range 2-31 years]; 54% male, 46% female). Pain was assessed using the Edmonton Symptom Assessment System (ESAS), PROMIS pain interference (PI) and pain behavior (PB), and PedsQL questionnaires scored according to their manuals. Descriptive statistics were used for analysis.
Results/Conclusions: The ESAS (age 13-31 years) mean total symptom distress score was 20.5±17.8 (mild). PROMIS parent proxy PI, PI, and PB (age 8-17 years) mean scores were 65.0±14.4 (moderate), 63.0±11.3 (moderate), and 56.9±13.0 (mild), respectively. PROMIS PI and PB (age 18-31 years) mean scores were 55.3±10.7 (mild) and 58.5±5.1 (mild), respectively. PedsQL parent (age 2-17 years) and self (age 5-31 years) mean total scores were 63.1±18.6 and 66.3±18.5, respectively (healthy=87.61±12.33). While limited by survey bias, these results support the presence of measurable pain at baseline in children and adults with OI, further demonstrating the need for adequate pain management in this population. Additionally, now that pain has been demonstrated at baseline, future studies should investigate how pain changes following treatment modalities, including bisphosphonates.
