Abstract

Despite the abundance of evidence that exists to support that screening and addressing social risk in patients improves health-related outcomes, implementation of screening within primary care practices has been inconsistent. In an effort to begin social risk screening in primary care, UPMC Central PA Medical Group moved forward with implementation of a standard screening tool imbedded within its electronic health record, Epic, in March 2023. Primary care providers and clinical staff members were charged with being the primary source of screening and referral management. In parallel to the medical group’s implementation, a centralized transitional care team began telephonically screening patients after acute care discharge- a population focused approach. The medical group has experienced a disconnect between the intent of the implementation and the outcome. Stakeholders, including providers and patients, are confused by the purpose of the implementation. The purpose of this study is to examine the implementation of social risk screening within UPMC Central Pa Medical group, provide evidence-based recommendations for improved implementation, and discuss how to use population specific social screening results to guide referral resource development and utilization.
