Abstract
This webinar will explore how organizations can help address the social determinants of health through the use of Community Health Workers (CHWs). Jeannine McMillan, MEd., the executive director of the 1889 Jefferson Center for Population Health, will share challenges, strategies, and successes. CHWs Justina Schreiber and Stephanie Buck will talk about their roles and how they help individuals and families.
