Description

Background

- ~1 in 5 women of reproductive age are obese, with bariatric surgery increasing in popularity as an intervention

- Gestational diabetes mellitus (GDM) is associated with shoulder dystocia, polyhydramnios, weight >4500g, hypoglycemia, hyperbilirubinemia, NICU admission, respiratory distress, cesarean delivery, gestational hypertension (HTN), and Preeclampsia

- Adverse outcomes of GDM can be mitigated with early diagnosis and blood glucose management

- The glucose tolerance test (GTT) is the current gold standard for diagnosing gestational diabetes (GDM)

  • Individuals with a history of bariatric surgery may not tolerate the sugar load or volume of the GTT
  • The GTT may not accurately predict GDM in patients with a history of bariatric surgery

- There are no current ACOG guidelines for GDM screening in this population
