Abstract

Objectives

1. Briefly review the "upstreamist" landscape and opportunities to improve health
2. Describe community health workers, their identity and training-- and contrast with other members oft he health care team
3. Review evidence that confirms the roles & effectiveness of community health workers in health care
4. Appreciate how a JFMA CHW contributes to interdisciplinary care that closes gaps and builds health
