Abstract
Objectives
By end of session, participants will be able to:
- Articulate the drivers of health outcomes and the impact of social "influencers" of health on clinical care in a primary care setting
- Describe the features of the Community Health Needs Assessment & Improvement Plan, including Jefferson's priority areas for 2019-2021
- Participate in generating ideas for one "tactic" that could be implemented in our clinical setting(s) and would address a priority area in the Jefferson CNHA
