Abstract

Objectives

At the end of this presentation, participants will be able to:

• Identify the goals of a successful transition from pediatric to adult care for adults with disabilities or complex chronic conditions that begin in childhood and recognize the core elements of that transition process
• Describe health disparities experienced by adults with disabilities
• Understand the outcome evidence for structured transition interventions
• Engage in a conversation about how we can improve care for this vulnerable population at Jefferson
