In support of the Know Diabetes by Heart™ (KDBH) initiative, the American Diabetes Association has assembled a collaboration of healthcare systems and stakeholders in Philadelphia and the mid-Atlantic region to improve population health outcomes for people with diabetes with or at risk of cardiovascular disease via its Diabetes INSIDE® quality improvement initiative.

The overall goal of this quality improvement (QI) initiative is to improve population health for people with diabetes as defined by the ADA Standards of Medical Care in Diabetes. The QI Initiative is a 3-year effort that engages healthcare systems with QI training, coaching and facilitation, data analysis, data-driven professional education, shared learning, and guideline support.
